  • Notify us of changes in health status, name, address, telephone numbers, email address, or insurance coverage.
  • Let us know when you see other health care providers so we can help to coordinate the best care for you.
  • Keep scheduled appointments or call to reschedule. Please cancel as early as possible.
  • Come to each visit with updates on medications, dietary supplements, or remedies you are using. Include any questions you may have.
  • Pay your share of fees incurred for health care services received.
