As a convenience to you, our insurance personnel will
bill your insurance company after receiving proper information and
signatures. An insurance identification card must be presented and
any co-payment must be collected prior to seeing the physician. We
accept American Express, MasterCard, Visa, personal check, or
cash. It is the patient's responsibility to be aware of any
restrictions on your individual plan.
