Terms of Service
We will happily file for insurance reimbursement with
any insurance provider with whom we are contracted upon payment of your
office co-pay. A current insurance identification card must be on
file.
Payment / insurance co-payment for services rendered
are due at your scheduled appointment. We accept American Express,
MasterCard, Visa, personal check, or cash.
24 hours notice of cancellation
is required
to avoid being charged for a missed appointment.
